Protecting Your Medical Information at Denver Audiology

This Notice explains how we may use or share your health information and how you can access it. It follows the rules of the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Please read it carefully.
Your Rights Under HIPAA
HIPAA gives you the right to limit how we use or share your protected health information (PHI) for treatment, payment, or healthcare operations.
What These Terms Mean:
- Treatment – We may use your information to manage or coordinate your hearing care. For example, we might share your health information with another provider to aid in your care. We will ask for your written permission before sharing your records outside our office, unless it's an emergency.
- Payment – We may share your information with your health plan to get paid for the services we provide. For example, we might send details of your care to your insurance company. We’ll ask for your written permission first.
- Healthcare Operations – We may use your information to enhance our services, train our staff, or evaluate the quality of care.
We may also use or share your information for:
- Health research
- Public health or safety
- Legal obligations
When Can We Use or Share Your Health Information
Here are some situations where we may use or disclose your medical information:
- To coordinate your care with doctors, nurses, audiologists, students, or lab services.
- To file insurance claims or verify coverage with your health plan.
- To talk about different treatment options or services—if we’re paid to do so, we’ll get your written consent unless it’s a face-to-face conversation or a small gift is involved.
- To a medical examiner for identification, cause of death, or organ donation.
- If you are in the military, the law requires us to share it.
- To public health or law enforcement to help prevent illness or injury.
- To the FDA to report product issues or side effects.
- To comply with Workers' Compensation rules.
- When ordered by a court or legal process.
- To contact you for fundraising, we’ll give you a way to opt out.
- For marketing or if we sell your health info, we’ll get your written permission first.
- When required by law, we’ll follow the law and only share the necessary information.
- To business associates who help us with services and have agreed to protect your privacy.
- To family members, caregivers, or legal representatives when needed for your care, unless you object. If you’re unavailable, we’ll share only the necessary details.
Your Privacy Rights
As a patient at Denver Audiology, you have the right to:
- Ask us to limit how we use or share your health information. We are not required to agree unless you paid for your care out of pocket and want us to avoid sharing it with your health plan. You must send this request in writing.
- Be informed of a data breach if your unsecured health information is compromised.
- Ask us to communicate with you in a specific way, like by email or at a different address. Send this request in writing.
- Get a copy of your medical records or this notice. Just ask the receptionist.
- Ask for changes to your medical records if you believe they are incorrect or incomplete. We may deny your request if we believe the records are already accurate. This request must be in writing.
- Request a list of who we’ve shared your medical information with for the last six years (excluding sharing for treatment, payment, or healthcare operations).
- Request a paper copy of this notice, even if you received it electronically. You must make this request in writing.
- Name someone to act for you, such as a legal guardian or someone with medical power of attorney. We’ll verify their authority before allowing access to your health information.
Our Privacy Responsibilities
At Denver Audiology, we are committed to protecting your medical information. We create and store health records to provide care, get payment, and manage our operations.
We are legally required to:
- Keep your health information private.
- Give you this notice explaining our privacy practices.
- Update this notice if our practices change. You may ask for a copy at any time.
If we change our privacy policies, we’ll update this notice. The new version will apply to all health information we have, including past records.
